The move marks one of the most extensive health data integration efforts in sub-Saharan Africa, as the country shifts from fragmented hospital records to a centralised, nationwide database designed for long-term digital and AI use.

At the centre of the rollout is SmartCare Pro, a government-led electronic health record system implemented through the Ministry of Health and SMART Zambia Institute. The platform now reportedly integrates over 12 million historical and active patient records across the national health network. It also connects more than 2,000 health facilities, allowing patient information to be accessed across different levels of care instead of being locked in individual hospitals or regional systems.

Officials say the system is not just about digitisation, but standardisation. By bringing multiple legacy databases into one structure, Zambia is creating a unified dataset that can follow patients across clinics, hospitals, and regions. This is a major shift in a system that previously relied heavily on paper-based records and disconnected digital tools that made nationwide data tracking difficult and inconsistent.

The government has also positioned the platform as “AI-ready” infrastructure, a term that refers to the system’s ability to support advanced analytics, predictive modelling, and machine learning tools in the future. While no nationwide AI diagnostic system is currently deployed, the structured nature of the database is designed to allow future technologies to analyse disease patterns, optimise resource allocation, and support clinical decision-making once layered on top of the existing system.

Beyond infrastructure, implementation has included large-scale workforce training, with reports indicating over 14,000 health workers trained to use the system, alongside adoption programs aimed at improving consistency across facilities. However, experts note that the transformation is still in its foundational phase: Zambia’s achievement is primarily a data consolidation milestone, with AI capabilities still in the preparatory stage rather than active clinical deployment.

Zambia’s 12-million-record consolidation is a verified national digital health milestone, representing a shift toward unified electronic healthcare infrastructure. The “AI-ready” label is accurate in terms of system design, but remains forward-looking rather than operational, signalling future potential rather than current AI-driven healthcare delivery.

Zambia has consolidated more than 12 million patient records into a unified national digital platform as part of a major health system upgrade built around its SmartCare Pro electronic health record system.
